BEAR WAREHOUSE: Form and Rules

The BEAR Warehouse form is now digital! Please scan the QR code once inside the BEAR Warehouse for easy online ordering and immediate pick-up!

Car seats, pack & plays, etc. may be ordered and picked-up at BEAR’s Warehouse
Monday – Friday, 9 am – 3 pm.

  1. Parents and/or Guardians are NOT ALLOWED at the BEAR Warehouse.
  2. Items in the BEAR Warehouse are available ONLY to ACTIVE CPS cases.
  3. Case information is REQUIRED before items are removed from the BEAR Warehouse.
  4. Items must be delivered to the client and not stored in CPS offices.
  5. Every child that receives resources from the BEAR Warehouse MUST be accounted for via our digital form. This form also includes a signature to verify that the resources being removed from the BEAR Warehouse are going directly to the children on the case and no one else.
